This commit replaces all header guards in DataHandling with #pragma once

Sums and averages are now calculated correctly, i.e. the masked bins are
ignored.

Also reworked the performance test which originally didn't time the
actual algorithm but the creation of the test workspaces.

Update to clang-format-5 and reformat the codebase 

It was blindly assumed that the InputWorkspaces has an axis of the type
SpectraAxis causing a segmentation fault. A fresh SpectraAxis is now
created for the input workspace independent of the original vertical
axis. Also:
- Added unit tests
- Updated sphinx documentation
- Updated release notes

from the input instructions, and another that takes the groups and
converts them to a map file format.
