Fix token refresh behavior for old access tokens

Summary of Changes

This makes two functional changes:

  1. I've removed the threshold at which we stop attempting to refresh old access tokens.
  2. I logout the user if their token refresh fails.

Need to figure out how to properly test 2, but once done I will assign someone here to review.
