Modify AlgorithmProxy::executeAsyncImpl to avoid creation of third, redundant thread when executing algorithms asynchronously (e.g. in MantidPlot). Also some tidying up of the affected files. Re #1333.

Refs #1267; fix build error on Windows; apparently 'or' compiles on linux or mac but you need the double pipe under windows.

Refs #1267; made the spectrum list dense from zero to getNumberHistograms(); filled out the spectraDetectorMap. Used a vector of pointers to event lists to avoid wasting memory and processor.

Disable ICat build. I don't have time to fix the Mac build now, which is picking up the openssl headers from Third_Party in preference to /usr/include. Re #1317.
