Fix various memory warnings and clang format

In places other substitutions have been made, e.g
Clang does not yet specialize std::shared_ptr for T[]. Vector
has been used instead. The operator[] methods were incorrectly
marked const but returning a non-const reference - this has been fixed.

Runs clang-tidy pass by ref across the codebase to fix-up various
warnings we are seeing in cppcheck

This is not necessarily one-to-one (although it is for all files to date).
If it is not, load it as follows:

"The spectrum spectrum_index[j] maps to detector_count[j] detectors.
Their detector IDs are in detector_list, starting at the index given by
detector_index[j]."
