plan: orchestration v2 redesign — post-dry-run

Synthesis of dry-run findings (Analyst F1-F5, Developer §3.1-§3.7,
Integrator §4.1-§4.3) plus transcript-level evidence into a
concrete plan for evolving the protocol. Addresses the four
questions raised in dry-run-transcript.md Prompt 1: stall
mitigation (typed STALLED events + ScheduleWakeup heartbeat +
(SHA, name) dedup), visibility (per-agent state file + statusline
+ recap suppression), the Administrator agent (subsumes
Initialization, adds runtime monitoring, never autonomously
restarts workers), and rate-limit alternatives (git ls-remote with
narrow refspec as v2 default; GitHub REST + ETag and SSH as opt-in).

Plan only — no edits to orchestration.md or the prompt files yet.
Phase 1 of the implementation plan (§10) lands those once the
human approves the redesign.
